2026 Ball Hockey Registration
If you would like to sign your child up for Ball Hockey please complete this form. In the past Mondays at 6pm has been U5-U7, 7pm and 8pm U13 and Wednesday nights 6pm & 7Pm U11 and 8pm U13. This is subject to change depending on final registration numbers. If you are unsure please sign up and send in your payment. If for some reason your player is not able to participate send us and email ([email protected]) and we will promptly send you your money back.

ROWAN'S LAW CONCUSSION ACKNOWLEDGEMENT

The Ontario Government has enacted Rowan's Law (Concussion Safety), 2018, S.O. 2018, c. 1 ("Act"). Ontario Regulation 161/19, the Act requires all sport organizations as defined in the Regulation ("Sports Organization"), which includes Stirling Blues Youth Ball Hockey(Stirling Blues Ball Hockey), to have a Concussion Code of Conduct. This Concussion Code of Conduct must require participants, as set out in the Act, to review the Ontario Government's issued Concussion Awareness Resources on an annual basis. A participant is subject to a Concussion Code of Conduct for each Sports Organization a participant is registered with.

The Stirling Blues Ball Hockey Concussion Code of Conduct and the appropriate Concussion Awareness Resources must be reviewed before you can register/participate in Stirling Blues Ball Hockey.

STIRLING BLUES YOUTH BALL HOCKEY CONCUSSION CODE OF CONDUCT
I will help prevent concussions by my commitment to:

  •  Wearing the proper equipment for my sport and wearing it correctly
  • Respecting the rules of my sport or activity.
  • Fair play and respect for all* (respecting other athletes, coaches, team trainers and officials).

I will care for my and others health and safety by taking concussions seriously, and I understand that:

  • A concussion is a brain injury that can have both short- and long-term effects.
  • A blow to the head, face or neck, or a blow to the body that causes the brain to move around inside the skull may cause a concussion.
  • A person doesn't need to lose consciousness to have had a concussion.

I will commit to:

  • Report any possible concussion received during participation in the MBHO to a designated person.
  • Recognizing a concussion or possible concussion and reporting to a designated person when an individual suspects that another individual may have sustained a concussion.
  • Sharing any pertinent information regarding incidents of a removal from sport with the Player's school and other sport organization with which the player has registered.
  • Sharing any pertinent information regarding incidents of a concussion that have occurred outside of participation in the Stirling Blues Ball Hockey to a designated person with your/individual's team.
  • Complete Injury Report Forms in a timely manner and ensure they are submitted to the League and/or Organization/Federation/Association.
  • Give commitment to providing opportunities before and after each training, practice and competition to enable participants to discuss potential issues related to concussions.
  • Maintain an open dialogue with all athletes and participants (and parents/guardians in cases of minors) about their health and any signs and symptoms of concussion they may experience.
  • I will commit to respect the Stirling Blues Ball Hockey Removal and Return to Play Protocol by:
  • Understanding that if I have a suspected concussion, I will be removed from sport and that I will not be able to return to training, practice or competition until I undergo a medical assessment by a medical doctor or nurse practitioner and have been medically cleared to return to training, practice or competition.
  • Understanding I will have to be medically cleared by a medical doctor or nurse practitioner before returning to training, practice or competition.
  • Respecting the roles and responsibilities of all coaches and health care professionals in Return to Play protocol.
  • Responding appropriately with Return to Play protocols if a participant is experiencing concussion related symptoms or if you suspect any participant has sustained a concussion.

RELEASE OF LIABILITY, WAIVER OF CLAIMS, ASSUMPTION OF ALL RISKS AND INDEMNITY AGREEMENT

I AGREE that I am over the age of 18 years (if under 18, parent or guardian agrees to the following on behalf of the player), and that I, the undersigned, agree that in consideration of myself being permitted to enter and use any one of the described lands, buildings, and premises used for ball hockey, and for ANY activities including, but not just limited to, ball hockey, on behalf of myself, my heirs, successors and assignors, DO HEREBY REMISE, RELEASE, INDEMNIFY, SAVE HARMLESS, DISCHARGE, AND FOREVER HOLD HARMLESS Stirling Blues Ball Hockey League, Stirling & District Recreation Centre, and the Township of Stirling-Rawdon, Stirling & District Minor Hockey Association, their directors, employees, volunteers, coaches, instructors, agents, and independent contractors and their heirs, successors, and assignors from any claims whatsoever arising by reason of any disease, deterioration of health, illness or injury to any person, including death, or for damage to, or loss of any of my property resulting from or arising from use of the lands and premises, from being present on the lands and premises, from participation in any program, from the use of any facilities or equipment located on the lands and premises, from acceptance of the advice of, or from the gross or will full negligence of the Stirling Blues Ball Hockey League, Stirling & District Recreation Centre, and the Township of Stirling-Rawdon, Stirling & District Minor Hockey Association Centre their directors, employees, volunteers, coaches, instructors, agents, independent contractors or any other persons using the lands and premises. The activities that I will be participating in will be inherently dangerous, and I will be exposed to risk of serious injury, disability, death, and risk of damage to or loss of property. I acknowledge that there may not be prompt access to medical assistance or treatment when participating in any activities, and I assume and accept any risk relating to the access to medical assistance and/or treatment. By signing this document, I acknowledge that I have read, understood and accepted the conditions of this waiver form as well as the rules attached as pertains to the league and are waiving certain legal rights, including the right to sue.

PRIVACY CONSENT

 

I do hereby consent to the collection and use of my personal information (including first and last name and hometown), personal images, athletic results and awards, prizes received, and verbal quotes, by posting on the website, or affiliated websites, of Stirling Blues Ball Hockey League, Stirling & District Recreation Centre, and the Township of Stirling-Rawdon, Stirling & District Minor Hockey Association  and/or by publishing in the newsletters of the following. I understand and agree that this information will not be sold to anyone without my prior written consent. I understand that my personal information can be viewed by anyone who accesses the abovementioned websites or publications, and that my consent can be withdrawn at any time, upon adequate prior written notice. I give this consent voluntarily and with the understanding that any of this information may be used in newspaper or magazine stories, posted on websites, and to verify my identity and registration in this event.
